Management of Ephelides and Solar Lentigines with IPL-Sq

Treatment

Ephelides and solar melanosis are benign pigmentary changes of the skin, resulting from a localized increase in melanin production induced mainly by exposure to ultraviolet (UV) radiation. Ephelides, popularly known as freckles, manifest as small hyperpigmented macules, predominantly in sun-exposed areas such as the face, neck, and back of the hands.


Solar melanosis, also called actinic lentigo or solar lentigo, is an acquired pigmented lesion associated with the progressive accumulation of actinic damage. It represents a clinical sign of chronic photodamage and skin aging induced by cumulative exposure to UV radiation.


Treatment of ephelides and melanosis generally involves a combined approach, including rigorous photoprotection, the use of topical lightening agents, and clinical procedures such as chemical peels, intense pulsed light (IPL), and pigment-specific lasers. Intense pulsed light (IPL) is based on the principle of selective photothermolysis, in which melanin acts as a target chromophore, absorbing light energy and promoting controlled heating and fragmentation of the pigment, favoring its progressive lightening.


The IPL-Sq® is an intense pulsed light handpiece compatible with the ETHEREA-MX® and ZYE® platforms. In this case, the patient underwent treatment with the ETHEREA-MX® platform with the IPL-Sq® handpiece, performing two sessions with a 20-day interval between them, without associated home treatment and using exclusively sunscreen (SPF).
